Falls Prevention Services

If you are referring or signposting someone to one of our funded services, please use the referral form linked below.

Available funded services:

• West Norfolk: 6 one-to-one personal training sessions.

• North Norfolk: 3 one-to-one personal training sessions.

Eligibility

To be eligible for these funded services, individuals must:

• Aged 65 years or over and at risk of falls.

• Socially isolated or unable to get out and about within the community.
