  • Anyone Aged 18+ who lives in Norfolk

  • Registered with a GP across Norfolk

  • A BMI equal or greater than 30

  • If your BMI is greater than 30 or 27.5 and above if you are of the Asian, Black African or African Caribbean community.

BMI = kg/m2 where kg is a person's weight in kilograms and m2 is their height in metres squared
